The Situation

Project Maven delivers machine-generated intelligence to every combatant commander by June 2026. During testing, AI target recognition misidentified at a rate of 12.3% in complex urban environments.

Golden Dome — the $185 billion missile defense architecture — places AI at the core of battle management. These systems must distinguish real warheads from decoys in seconds. No verification architecture exists for these decisions.

On February 27, 2026, the Department designated its only governance-requiring AI provider a supply chain risk. Hours later, a $200 million contract was awarded to a company whose CEO described the deal as "rushed" and "sloppy."

The NDAA mandates a governance framework by 2027. No organization has been resourced to build it.

Beyond Defense

Healthcare: the leading sepsis prediction algorithm missed 67% of cases across major hospital systems. A population health algorithm systematically deprioritized Black patients, affecting 200 million encounters annually. ECRI ranked AI the number one health technology hazard two consecutive years.

Finance: a single algorithm error lost $440 million in 45 minutes. Automated rebalancing triggered a UK gilt market spiral requiring £65 billion in emergency Bank of England intervention.

The failure is not limited to defense.

The Adversary

In September 2025, a Chinese state-sponsored group used AI to perform 80–90% of the operational workload in a campaign targeting 30+ organizations across technology, finance, government, healthcare, and critical infrastructure. AI-enabled cyberattacks increased 89% year-over-year. China filed 9,000+ PLA AI procurement requests between 2023 and 2024.

Russia deployed the Lancet loitering munition with AI autonomous targeting in active combat in Ukraine. Israel's Lavender system listed 37,000 targets. Officers reviewed each in an average of 20 seconds.

These systems operate without structural governance. So do ours.
